The Office of Inspector General of the Department of Health and Human Services (“OIG”) released an on-line Provider Self-Disclosure Protocol (“Protocol”) submission form for individuals and entities that seek to disclose to the OIG potential violations of criminal, civil, or administrative law governing health care programs for which exclusion or civil monetary penalties are authorized. The Protocol, first made available in October 1998, is a key reporting process for health care providers to use, as appropriate, to resolve matters in a cooperative manner with the OIG. The electronic form does not preclude the option of a written submission and all electronic submissions must include the elements required in the Protocol.
